Any Dentist, Anywhere, Fitness First Healthcare As a member you have the opportunity to participate in our Dental Indemnity Benefit. There is no panel of dentists, so you have the freedom to select the provider of your choice. As a member you will know exactly what the plan pays for each procedure. The difference between the scheduled allowance for a procedure and the fee charged by the dentist, is paid by the patient. You also have the option to choose whether payment of claims is made directly to you or to the dentist. Example: My dentist charges $30 for a routine dental cleaning.  The dental benefit is $18, so I will pay my dentist just $12 to have my teeth cleaned after I've met my annual $50 deductible. Members receive the maximum savings when the Dental Indemnity benefit is used in conjunction with the discounts from the Dental Discount benefit.     *Please note that the prices and savings will vary by provider and the actual products or services purchased.   Certain Fitness First Healthcare membership programs include some limited insurance coverages.

Deductible & Benefit There is a $50 deductible required per person each calendar year. After the deductible is met, the plan pays a maximum benefit of $500 per person per calendar year. Waiting Periods Preventive procedures are covered immediately Basic procedures subject to ninety (90) day waiting period Major procedures subject to twenty four (24) month waiting period This is not an insurance certificate. The certificate contains a summary of the coverage with a complete list of covered charges, exclusions, and limitations. This Description of Benefits explains only the general purpose of the insurance described, but in no way changes or effects the insurance afforded under the Master Policy.
