With the Co-Pay Vision Benefit, members have the option of obtaining services from a nationwide network of participating eye care professionals consisting of optometrists, ophthalmologists and opticians who deliver quality patient care. From eye exams to laser eye surgery, these professionals meet the evolving needs of our members. $0 Co-Pay on Eye Exams! One annually for each family member. $0 co-pay when utilizing a participating network provider doctor.  When visiting out-of-network providers, the maximum covered expense toward an eye exam is $47.00.
